MISSION
The Support You Need
The mission of the Rancho Cordova Adult Day Health Center is to provide a community-based location that promotes the social, emotional,and physical well being of disabled adults and elderly persons, and to serve as a liaison between the existing health care systems.
We strive to decrease hospitalization and emergency room visits, minimise isolation and depressive episodes, and increase mental stimulation.
SERVICES
WHAT WE DO
Rancho Cordova Adult Day Health Care Center has a multidisciplinary team of health professionals that conduct a comprehensive assessment of each potential participant to determine which ADHC services meet the individual's specific health and social needs. Each participant is provided an individualised plan of care which may include any of the following required services:
* Medical and Nursing Services
*Social Services
*Psychological Services
*Physical Therapy
*Speech Therapy
*Occupational Therapy
*Recreation and Social Activities
*Meals,Snacks & Nutritional Counseling
*Transportation to/from home
FACILITIES
ADHC is for frail elderly persons and other impaired adults, ages 18 and older, who meet any of the following criteria:
* Frail elderly and others who need individual and rehabilitative healthcare.
*Residing in community alone and in danger of institutionalization.
*Discharge from a general acute care hospital and otherwise placed in a long-term care institution.
*Resident of a nursing facility or other long-term care facility, but is determined that placement is unnecessary.
What is the Cost?
*Adult Day Health Care(ADHC) is covered by Medi-Cal benefits.
*Some private insurance companies may cover the cost of ADHC.
* All Medi-Cal recipients are welcome.
* Private pay is on a Non Medi-Cal Fee Schedule.
*Veteran Approved
